Il lutto perinatale

Il lutto perinatale rispetto a qualsiasi altro tipo di lutto ha in più “l’aggravante” di essere inaspettato e spesso inspiegabile e di colpire la coppia genitoriale nel momento in cui sta portando avanti il progetto di creare una nuova vita e quindi si appresta a festeggiare una nascita. Ha quindi una forte connotazione traumatica.
Questo lutto è quindi particolarmente crudele in quanto sovverte le aspettative anche biologiche legate all’atto di mettere al mondo un figlio, che in questo caso si traducono in un atto di morte invece che di vita. La percezione della donna è di aver dato la morte invece che la vita.

L’intensità del dolore legato al lutto può variare in riferimento a molti fattori: lo stadio della gravidanza in cui avviene, la storia pregressa della coppia, il sostegno e le cure ricevute in ospedale e al rientro a casa. Si è portati a ritenere che la perdita del bambino nelle prime settimane di gravidanza possa essere meno traumatica rispetto a morti avvenute in epoche gestazionali più avanzate, ma è dimostrato che il fattore maggiormente correlato all’intensità della sofferenza è il coinvolgimento emotivo e l’investimento affettivo della coppia.

La sofferenza rende il lutto qualcosa di tangibile e conduce gradualmente alla sua accettazione. Convenzionalmente l’elaborazione del lutto attraversa diversi fasi e/o momenti, che hanno una durata variabile

  1. Fase dello shock e della negazione: in cui il genitore non si capacita di ciò che è successo e reagisce meccanicamente. Si manifestano sentimenti di disorientamento, confusione, senso di colpa, accompagnati al tentativo di trovare cause oggettive per spiegare ciò che è accaduto.
  2. Fase della rabbia: c’è il manifestarsi di emozioni forti quali rabbia e paura, che esplodono in tutte le direzioni
    3. Fase della depressione: disorganizzazione che porta all’isolamento e alla solitudine. C’è una perdita degli interessi verso il mondo esterno accompagnata ad un sentimento di essere incompresi da tutti.
    4. Fase dell’accettazione: con tempi e modalità che variano da individuo ad individuo si assiste ad un graduale ritorno alla normalità, che passa dall’accettazione di quanto accaduto e dalla possibilità, in stadi di elaborazione dell’esperienza avanzati, di ricomprendere questo evento traumatico all’interno della propria esperienza di vita.

 

Al fine di attraversare nel migliore dei modi il lutto è possibile:

– Dare una degna sepoltura al bambino/a
– Riuscire ad avere una diagnosi certa della causa della mortlità del babmbino
– Frequentare gruppi di auto mutuoaiuto online o in presenza
– Rivolgersi a counselor, psicologo o psicoterapeuta

In ospedale puoi chiedere di:

  • tornare a casa se l’induzione del parto non avviene subito
  • essere informata sulle differenze- anche psicologiche, ormonali e ai fini dell’elaborazione del lutto, tra parto normale e cesareo
  • chiedere che il tuo bambino/a sia chiamato/a per nome
  • chiedere di vedere il tuo bambino (se non ti senti subito puoi farlo dopo qualche ora; non rinunciare è l’unica occasione in cui puoi conoscerlo) e se le condizioni del piccolo lo permettono puoi tenerlo in braccio il tuo vestirlo/a, fare delle foto, prendere impronte piedini e manine
  • chiedere che il bambino/a possa essere visto anche dai tuoi congiunti chiedere al personale un memory box dove poter mettere vestiti foto quaderno con le sue misure. Nel caso l’ospedale non ne avesse puoi farti portare dai tuoi congiunti un quadernetto ecc.
  • Hai diritto a chiedere la sepoltura e a chiedere la consegna del corpo del bambino/a per sepoltura o cremazione del bambino. Maggiori Informazioni La normativa per la sepoltura del feto
  • chiedere al personale se è possibile avere una camera lontana dalle altre mamme che hanno appena avuto il loro bambino
  • chiedere di essere appoggiata nel più breve tempo possibile a operatori che sul territorio trattano il tema della perinatalità e presso i quali potrai incontrare altri genitori in lutto

Al ritorno al casa:

a) Concediti tutto il tempo per piangere il tuo bambino
b) Misura i contatti sociali in proporzione alle tue forze
c) Molto probabilmente non ti sentirai capita ed è “normale” che sia così: gli altri non solo non hanno provato ma non parole adeguate
d) Proverai un dolore fortissimo da cui ti sembrerà quasi impossibile uscire: anche questo è “normale”. Perdere un figlio è il dolore più grande che si può provare
e) Appena ti senti prendi contatto con associazioni e professionisti che trattano il tema
f) Se non ti senti di uscire usa i servizi online (forum) che molte associazioni hanno attivato

The perinatal mourning, compared to any other kinds of mourning, is worse because it’s unexpected and unexplainable and it hits the parents in the moment in which they are trying to create a new life. It has, therefore, a more traumatic characteristic. This mourning is especially cruel since it overthrows even the biological expectation connected with the act of giving birth to a child, that, in this case, are translated in an act of death instead of life.
The perception of the woman is have given death instead of life.

The intensity of the suffering connected to the mourning can be different based on many factors: the advancement of the pregnancy, the history of the couple, the comprehension and the care received in the hospital and upon returning home. We are used to thinking that the loss of a baby in the first weeks of pregnancy is less traumatic than the loss in a more advanced state of the pregnancy, but it has been demonstrated that the fact is more connected to the intensity of the suffering of the emotional involvement and investment of the couple.

The suffering makes the loss something tangible and drives gradually to its acceptance.
The processing of the mourning goes through different stages and moments, that are variable in time.

  1. The stage of shock and denial: a parent is not able to understand what happened and react mechanically. They feel disorientated, confused, guilty, and they will try to find out the objective causes to explain all that had happened.
  2. The stage of anger: the manifestation of strong emotions like anger and fear that explodes in all direction
  3. The stage of depression: disorganization that brings to isolation and loneliness. There is a loss of interests towards the outside world with a feeling of being misunderstood by everyone
  4. The stage of acceptance: with time and ways that are different for each person you can see a gradual return to normality, that goes though the acceptance of what happened and the possibility, in the advanced stage of the processing, to understand that this traumatic event is inside your own experience of life.

 

What to do to get through the mourning in the best possible way:
– Give a proper burial to the child
– Manage to get a sure diagnosis on the reason of the child’s death
– Attend support groups online or in person
– Ask a counselor, psychologist or psychotherapist for help

At the hospital you can ask to:

  • Go back home if they don’t induced labor right away
  • Get information on the differences between natural birth and having a cesarean and their psychological, and hormonal consequences
  • Ask that your child be called by name
  • Ask to see your child (if you don’t feel like doing it at moment you can do it after a few hours: don’t give up this chance it’s the only one you have to get to know them) if the condition allows you, you can hold them, get them dressed, take pictures, take a stamp of the feet and the hands
  • Ask that the child be seen by your family
  • Ask to the sanitary personal to have a memory box where you can put the stamp, the pictures, and dresses. In case the hospital doesn’t have one ask your family to bring it
  • You have the right to ask about the burial and ask that the child be delivered for burial or cremation. Ask major information about the law for the burial of the fetus
  • Ask to be put in a room far from the other mothers that just had their baby
  • Ask to be put in contact with a specialist in perinatal in your area that will get you in touch with other parents that are in mourning

Back at home:

a) Give yourself time to cry your child
b) Control your social encounters to what you’re able to deal with
c) Very likely you will feel that you are not understood, that is “normal”: the others not only didn’t go thought what you did but they also don’t have the right words.
d) You feel a pain that it will seem almost impossible to get through: even this is “normal”. Losing a baby is the worst kind suffering that can be felt
e) As soon as you feel ready, get in touch with the association and a professional that can help you
f) If you don’t feel like going out, use the online service (forum) that many associations have
